FOUR (Full Outline of UnResponsiveness) Scale

A clinical tool for assessing the level of consciousness in patients with acute brain injuries. It consists of four components: eye response, motor response, stem reflexes, and breathing patterns, each of which is rated from 0 to 4 points, giving a maximum of 16 points. Unlike the Glasgow Coma Score, the FOUR score takes into account stem function and respiration assessments, making it useful for intubated patients in whom it is not possible to assess speech response. It is often used in neuroresuscitation to monitor the neurological condition and identify changes that require urgent action.
